Admission to Henry County Health and Rehabilitation is based on bed availability and our ability to meet the needs of your loved one. We appreciate your inquiry into our services. If you would like to place your loved one on the waiting list, please call our admissions office. Our facility accepts admissions from the hospital, home, or other facilities. Every effort will be made by our staff to ensure that the transition to our facility is as smooth as possible. The following will give you an idea of this process:

The typical admission process is as follows:

1. Placement on waiting list for a bed.
2. Submission of all medical records requested.
3. Review by nursing home staff.
4. Discussion of financial arrangements.
5. Notification of bed opening.

Once you are notified of the bed availability and accept, we request that you bring the following with you on the day of the admission.

1. Resident's Medicare and Medicaid card
2. Resident's Social Security card
3. Supplemental Insurance Card if applicable (ie: BC/BS, Ct, etc.)
4. Power of Attorney (if applicable)
5. Living Will (if applicable)
6. Clothing- four to five changes
7. Personal effects- calendar, clock, pictures, figurines, etc.
8. Current medications.
9. Medicare Part D drug card.
